Exercise to Manage Pain Due to Osteoarthritis in the Elderly

Osteoarthritis (OA) is a prevalent joint condition among elderly individuals, characterized by pain, stiffness, and reduced mobility. OA is most common life style disease in people over 65 years of age, which is more common than high BP and diabetes. The implications of OA extend beyond physical discomfort, impacting daily activities, independence, and overall quality of life. Over 2 million Australians have hip or knee osteoarthritis and there were 1.5 million consultations at the GP each year. 

Why should you be physically active? 

  • The answer is simple. Physical activity is medicine, which reduces the risk of getting inactivity-related disease; cardiovascular disease, diabetes, dementia and certain cancers. There are numerous benefits from being physically active from exercising.
  • Lowers blood pressure
  • Improves the way your body handles blood lipids/cholesterol and blood sugar levels.
  • Better physical fitness will reduce how tired you feel after exercise and your body will use less energy to do the same amount of work.
  • Bone density will increase with gradually loaded exercising.
  • Cartilage requires dynamic load to regenerate 

Cartilage needs to be appropriately loaded to regenerate itself!

  • When loaded and unloaded, synovial fluid is pumped in and out of the cartilage. This stimulates new development of cartilage which improves its shock absorbing capacity. Starting to exercise early will have a greater effect on the development of cartilage compared to if you start when you already have severe stages of OA. 

Specific rehabilitation approaches for OA include gradual loading of joints, which involves gently increasing the amount of weight or resistance placed on the joint over time. This approach helps improve the functional level of the joint and allows patients to participate in more physical activities safely. 

The GLAD Program: 

One effective exercise and education program for OA management is the GLAD (Good Life with Arthritis Denmark) program. This evidence-based program combines strengthening and flexibility exercises with education on OA management strategies. Participants learn self-management techniques, including pain management, joint protection, and lifestyle modifications, to support their overall health and well-being. 

GLAD certified Physiotherapists play a crucial role in implementing the GLAD program, guiding participants through exercises and providing education on OA management. They ensure exercises are tailored to individual needs and capabilities, making adjustments as needed to maximize benefits and safety.
